Form A (Beneficial Owner Declaration)

Form A is a declaration filed with the UAE's Ministry of Economy or the relevant economic department under the beneficial ownership regime (Cabinet Decision No. 58 of 2020 and its amendments). It identifies the ultimate natural person or persons who ultimately own or control a legal entity registered on the mainland or in most free zones — by ownership of 25% or more of shares, voting rights, the right to appoint or remove managers, or other means. The form requires name, nationality, date and place of birth, national ID, address, and the basis of control. Form A must be kept current and filed with the licensing authority; failure to do so exposes the entity and its officers to fines and licence suspension. DIFC and ADGM have their own equivalent UBO disclosure regimes.
